Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, 2) signed online visa request form submission
sheet with one photo, 3) business letter, 4) photocopy of driver's license, 5) a copy of printed online order
form, 6) your payment (Credit Card Payment Form must be enclosed if you pay by credit card), and 7) a
prepaid airbill or envelope for return shipping (optional), to the following address:

Return Shipping Fees Per Package
We process Brazil visa applications
for residents of the following states: Arkansas,
Colorado, Kansas, Louisiana, New Mexico, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 2 blank visa pages.
Non-US citizen must submit a copy of permanent resident card or valid U.S. visa.
Visa Request Form and Photo
Complete and submit visa request form online. Print out the submission sheet after you finish. Sign it
and attach one standard passport photo. Send the submission sheet with your passport.
Business Letter
Send a business letter from the employer or sponsoring company stating 1) nature of business of the
U.S. company; 2) applicant's title and/or functions with the U.S. company and his/her precise nature of
business and activities to be conducted in Brazil; 3) responsibility of the U.S. company for applicant's stay
in Brazil and his/her return to the country of origin; 4) name(s) and address(es) of Brazilian company(ies)
with which applicant is going to deal/negotiate with; 5) name(s) of person(s) to be contacted in Brazil,
phone number(s) and his/her/their functions with the Brazilian company(ies); 6) expected duration of stay
of his/her immediate trip to Brazil. Must state that no service or technical support will be provided.
Proof of Residence
Send a photocopy of your driver's license.
