Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, 2) two copies of submission sheet with photos,
3) proof of residence and good conduct, 4) enrollment letter and financial proof, 5) a copy of printed
online order form, 6) your payment (Credit Card Payment Form must be enclosed if you pay by credit
card), and 7) a prepaid airbill or envelope for return shipping (optional), to the following address:

Return Shipping Fees Per Package
We process Brazil
visa applications for residents of the following states: Arkansas,
Colorado, Kansas, Louisiana, New Mexico, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 2 blank visa pages.
Non-US citizen must submit a copy of permanent resident card or valid U.S. visa.
Visa Request Form and Photo
Complete and submit visa request form online. Print out one copy of the submission sheet. Sign it
and attach one standard passport photo. Send the signed submission sheet with your passport.
Proof of Residence and Good Conduct Record
Send a proof of residence in the consular jurisdiction (Arkansas, Colorado, Kansas, Louisiana, New
Mexico, Oklahoma, and Texas) for the past twelve months, i.e., a photocopy of your driver's license.
A no-criminal record issued by the local police department is also required.
Proof of Enrollment and Financial Capability
Send notarized enrollment letter indicating duration of studies at a Brazilian educational institution and
copy of scholarship documentation or proof of financial capability to pay for the stay in Brazil.
Proof of Departure
A proof of departure is NOT required for getting a student visa.
