Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, 2) signed online visa request form submission
sheet with one photo, 3) travel itinerary, 4) photocopy of driver's license, 5) a copy of printed online order
form, 6) your payment (Credit Card Payment Form must be enclosed if you pay by credit card), and 7) a
prepaid airbill or envelope for return shipping (optional), to the following address:

We process Brazil visa applications
for residents of the following states: Arkansas,
Colorado, Kansas, Louisiana, New Mexico, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 2 blank visa pages.
Non-US citizen must submit a copy of permanent resident card or valid U.S. visa.
Visa Request Form and Photo
Complete and submit visa request form online. Print out the submission sheet after you finish. Sign it
and attach one standard passport photo. Send the submission sheet with your passport.
For applicants under the age of 18, the submission sheet must be signed by both parents and the
signatures must be notarized. Additionally, a photocopy of the child's original or certified birth certificate,
and a signed and notarized letter from the child's parent(s) who is not traveling with the child to authorize
the child to travel to Brazil are also required. Click here for the authorization letter form.
For applicants of the age of 3 months to 6 years old, a certificate of vaccination against polio is required.
Proof of Departure and Residence
Send a copy of your travel itinerary or airline ticket, and a photocopy of your driver's license.
