You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, 2) application form with one photo attached, 3) a
photocopy of Foreign Student Visa Application Form and Admission Letter, 4) a copy of printed online
order form, 5) your payment (Credit Card Payment Form must be enclosed if you pay by credit card), and
6) a prepaid airbill or envelope for return shipping (optional), to the following address:

The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 1 blank visa page.

Send one visa application form, completed and signed, with one standard passport photo.

Student Visa Application Form and Admission Letter
Submit a photocopy of an approved Foreign Student Visa Application Form (JW101 or JW202) and a
photocopy of your Admission Letter. Keep the original copy for yourself.
