or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.

Send the above documents, including 1) your passport, 2) application form with one photo attached, 3) a
copy of printed online order form, 4) your payment (Credit Card Payment Form must be enclosed if you pay
by credit card), and 5) a prepaid airbill or envelope for return shipping (optional), to the following address:

Return Shipping Fees Per Package

Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 1 blank visa page.

for a China visa for the first time or with a new passport, or a child who was born in U.S. with at least one
parent born in China and is applying for a China visa for the first time or with a new passport.
