You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, 2) application form with one photo attached, 3) a
photocopy of Visa Notification Form and a photocopy of Work Permit for Aliens or Foreign Expert's
License, 4) a copy of printed online order form, 5) your payment (Credit Card Payment Form must be
enclosed if you pay by credit card), and 6) a prepaid airbill or envelope for return shipping (optional), to

The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 1 blank visa page.

Submit a photocopy of Visa Notification Form and a photocopy of Work Permit for Aliens issued by
Chinese Ministry of Labor or a photocopy of Foreign Expert's License issued by Chinese Foreign Expert
Bureau. Keep the original copy for yourself.
