Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card. Credit card payment is subject to a 3% processing charge.
Shipping
Send the above documents, including 1) your passport, 2) application form with two photos, 3) a copy of
driver's license, 4) a business letter and an invitation, 5) a copy of printed online order form, 6) your
payment (Credit Card Payment Form must be enclosed if you pay by credit card), and 7) a prepaid airbill
or envelope for return shipping (optional), to the following address:

Return Shipping Fees Per Package
We process India visa applications
for residents of the following states: Alabama,
Arkansas, Florida, Georgia, Kansas, Louisiana, Mississippi, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 2 blank visa pages.

applying for a visa for the first time or with a new passport.
Application Form and Photos
Send one visa application form, completed and signed, with two standard passport photos. The
application form must be completed and printed online using the link given next.

Proof of Residency and Copy of Birth Certificate
Send a residence proof such as copy of your driver's license, other state issued ID or a utility bill, and a copy of birth certificate.
Business Letter and Invitation
A letter from your company indicating the business purpose of your visit, probable duration of stay, validity
of visa, places and organizations to be visited and a financial guarantee, and an invitation letter from a
company in India, should be submitted with your application. The validity of visa to be requested (1 year,
5 years or 10 years) should be clearly stated in the letter. Both business letter and invitation letter must
be written on company letterhead.
